Healthcare organizations invest enormous time, money, and effort into maintaining and adapting EMR systems. Yet despite that investment, change remains slow, expensive, and difficult. This is the central paradox: EMRs are indispensable, but they also limit healthcare’s ability to evolve.

Most of the major challenges facing EMR vendors — reducing costs, shortening long and complex implementations, enabling continuous updates, achieving true interoperability, scaling innovation, and leveraging AI in healthcare — are not local, but occur across countries and institutions. They are not vendor-specific, but affect both legacy and modern architectures alike. They are not temporary, but persist long after deployment. Systems often cannot evolve sufficiently after go-live, especially as clinical reality changes.
